Do many followers make a slow stream?
I have the same problem, it sometimes takes up to 30 seconds until the stream is there.
Do we have "the" optimal settings for such situations? E.g. reduce followers?

Best you can do is "spread the load" by setting the delivery interval higher and space out the deliveries. On shared hosting we can't adjust the size of the fpm worker pool and ActivityPub is really chatty and performs an insane number of fetches for each message processed. I just need to run a larger server, but financial turbulence and rocky employment has made me glad that I can still keep a half dozen modest fediverse servers running for around $5/month.
